    The studies show potential but just tell me that currently more robust clinical trials are needed. I like the use of sham cupping as a placebo. The problem with cupping and comparing it to placebo is that there seems to be no good placebo to compare it to. Unlike medications where one can really blind a study by giving a patient a pill that looks exactly like the test drug, it seems that one can't fool a test subject into getting something that looks like cupping but isn't? Patient might still be able to tell the difference based on technique and lack of bruising if a sham cupping technique is used. It seems hard to double-blind in short opening this up to so much bias.

    So, my cholesterol has been around 230 my whole life. I'm 65. Always the doctors wanted to put me on statins but I am afraid of them. I told my doctor that if the arteries around my heart were clogged up I would take the statin. She ordered a cardiac MRI. The tests came back and showed only one artery had a tiny bit of build up. Not enough to scare me into taking a statin. What I want to know is HOW DID THEY (WHOEVER THEY ARE) come up with what are acceptable numbers? What is normal for me isn't necessarily normal for you. I mean, my internal temperature, my whole life, is 97.2 degrees. "They" say normal is 98.6. Because that's what most people's temperature is. If 1 out of 4 people in the United States (that's 25%!) need to take statins to lower their cholesterol, maybe there is something wrong with their numbers. Let's say they're right. Surely they can do better, we deserve better. They make billions of dollars each year just off of the statin drugs! They need to go back to R & D and fix it! The cure shouldn't be worse than the ailment.

    When you talk about a full rotator cuff tear, I presume it's not all the way cut off in the width direction, just at some point somewhere it's cut off all the way in the height direction? Because that would mean that the tendon is still connected and then I can understand some people go through life without pain. If it would be totally detached I also would not understand how just therapy can make it better because the tendon would have to grow 'through the air', land on the bone and reattach itself.

    Dr. Ping you are quite right in criticizing the deficiency of these studies on type II collagen. However, we must consider risks versus benefit. If a person is facing the risk of joint replacement surgery, and add the risk of age influencing the first risk and compares that to the risk of shelling out a little cash to buy a supplement that is unproven, which may have benefit, or may not, it seems to me that the risk to benefit ratio of the latter is clearly much smaller than the risk to benefit ratio of the joint replacement surgery. So each person must decide for themselves of course, but if I have a possible treatment that has essentially no risk, i.e. collagen, and there is a potential benefit, then, inverting the risk/benefit ratio to a benefit/risk ratio, we have a number, comparing possible benefit divided by zero risk. The benefit/risk ratio then approaches infinity! It seems like a no-brainer! Why not go for it? We can be our own experiment! Douglas Palenschat MD, FACS

    I have developed arthritis in my right knee. Orthopedic knee sleeves allow me to walk for up to a few hours, but running, dancing, climbing stairs causes pain. I've been exercising but it brings little effect. I wish I could hike the mountains again, but it looks like ill never be able to do it again

    Dude, there are any number of studies that demonstrate D's anti-inflammatory effects. I don't know what study you are quoting, but to say D is ineffective in this regard is absurd. Perhaps it was one of the many underpowered D studies that (typically) give the patient ~800 IU a day. After a few weeks, when nothing happens, they reach the conclusion that "VD has no effect on condition X." But studies that give a functional amount (many thousands of IU a day, thank you!) very, VERY often show a positive impact. And the theory is there to justify the results! Those meta analyses you like are so often a conglobulation of basic bad research. Read at your own peril. FWIW, I stumbled upon D at the beginning of the pandemic. Not wanting to die of ARDS, I started taking 8K IU D3 a day, plus deliberate sunbathing at high UV-index times 2 or three times a week. (near-full body UV exposure can make 10,000, even 20,000 IU in your skin in ~30 minutes.) I expected nothing other than to hopefully reduce Covid risk. But then I woke up about a month later and realized that my long suffering OA hip and injured arthritic ankle suddenly didn't hurt any more. At all!! My sore back and deep upper and lower leg aches (classic D deficiency) also evaporated. Now, 4 years later, the hip that once had me limping 1/4 mile at a time is 'transparent' and good for as many miles as I can muster. The ankle is still slightly tender at times, but is 95% improved. After reading quite a lot of research articles on D and immune function, I'm sold on it as a broad anti-inflammatory and immune system modulator. Even my mesquite pollen allergies are vastly attenuated.

    I'm sure it's difficult to give "the in-depth truth" when you're profiting from the intervention. So, I'll fill in the gaps in studies that you forgot to mention (just a few of many more): JOINTS: pubmed.ncbi.nlm.nih.gov/38323999/ - Compared exercise to PRP to exercise + PRP on 157 patients with knee OA. Conclusion: "PRP did not improve pain at 24 weeks of follow-up in patients with mild-to-moderate knee osteoarthritis compared with exercise alone. Moreover, exercise alone was clinically superior to PRP alone, considering function and the physical component of health-related quality of life. Despite the additional costs and endeavors related to PRP products, the combination of PRP and exercise did not differ from exercise alone. The results of this randomized controlled trial do not support the use of PRP injections in the treatment of patients diagnosed with mild-to-moderate knee osteoarthritis. Consequently, exercise alone is the recommended treatment for reducing pain and enhancing function throughout this timeframe." pubmed.ncbi.nlm.nih.gov/32829298/ - Systematic review and meta-analysis of 11 different studies. Conclusion: "This systematic review and meta-analysis suggests that the short- and long-term clinical outcomes after intra-articular FJIs with normal saline and other active substances in patients with LBP are comparable" TENDINOPATHY: www.ncbi.nlm.nih.gov/pmc/articles/PMC8753395/ - The title says it all: "Platelet-rich plasma injection for tennis elbow: did it ever work?" pubmed.ncbi.nlm.nih.gov/34590307/ - Cochrane review of PRP for tennis elbow (lateral epicondylopathy). Conclusion: "With no evidence of benefit, the costs and risks are not justified." pubmed.ncbi.nlm.nih.gov/26198389/ - Systematic review and meta-analysis for hamstring injuries. Conclusion: "Meta-analysis showed superior efficacy for rehabilitation exercises. PRP injection had no effect on acute hamstring injury." www.ncbi.nlm.nih.gov/pmc/articles/PMC9066802/ - Systematic review and meta-analysis of PRP for patellar tendinopathy. Conclusion: "the PRP injection did not provide significantly greater clinical benefit than Non-PRP injections in the treatment of patellar tendinopathy." SAFETY: You claim that it's safe because you're injecting "your own cells". I don't think you understand the immune system. Getting your immune system on hyperdrive to attack it's own tissue is something called autoimmunity. PRP has the potential to drive that. Many studies warn against this. It is especially important in diseases which have a relapsing and remitting pattern.” prptreatmentbeverlyhills.com/side-effects-of-platelet-rich-plasma-injections/ - Not a study, but someone selling the intervention who is honest about it: “People with certain medical conditions, such as cancer or autoimmune diseases, may not be good candidates for PRP treatment.
